Home from Hospital

Patient Aftercare Scheme: Reading, Wokingham, and Newbury

Helping patients when they return home

 

This help is available to older people who feel they may need additional support after discharge from hospital.  With the help of trained volunteers, we provide short-term social and practical support, helping you to regain your independence on your return home.   We can check that your power and heating are in order, assist with your shopping, pensions, meals, etc., let people know you are home, and generally help you settle back into your routine. 

This service is available for up to 3 days after your discharge from hospital, and is free.

The aim of the scheme

Who can have this help?

By providing a "safety net" for patients for the first 72 hours after discharge, the scheme aims to:

  • prevent delayed discharge

  • help the patient regain their confidence

  • reduce the risk of re-admission.

The service is available for people who:

  • are due to be discharged from hospital

  • are resident in Berkshire

  • are over 60 years of age

  • were independent prior to being admitted, and who are medically fit on discharge, but would like extra help on returning home.

How do I get help under the scheme?

Not included in the scheme:

If you are due to go into hospital, and would like assistance when you are discharged, please contact the Home from Hospital Coordinator at Age Concern Berkshire.  If you are already in hospital, ask the ward staff, who will make a referral for you.  Your GP, friends, or family may also refer you under the scheme.

We cannot:

Perform any personal care tasks

Administer medication or change dressings

Offer long term befriending

Act as a substitute for statutory services
